Privacy Notice:
Your personal information will be kept in accordance with the Data Protection legislation on the basis that you have given your consent to information about you being held, which means that I will:

  1. Collect and process personal information (name and contact details); financial information (through bank transfers or online payments); and case notes and records (to carry out the agreed work).

  2. Use your contact details only to get in touch with you about matters relating to your therapy, such as appointments and to provide helpful information, where appropriate, unless you have given your explicit consent for me to contact you for other purposes.

  3. Retain notes of your treatment for a period of 7 years, in accordance with professional requirements and will take steps to ensure the security of the record.

  4. Not share your personal information with other individuals or organisations, except where I have reason to believe you or others to be at risk, or where there is a legal duty to disclose it, or in relation to payments made by you/ your bank for contracted services.

  5. Provide you with free access to the information I hold about you, should you wish.

Questionnaires or Outcome measures
I use various measures to gauge your response to treatment. If you are happy to do so, you will be asked to fill out some brief questionnaires before and after selected therapy sessions. These questionnaires help me understand your needs, your past and present strengths and how best to help.

Your personal details and the information from the questionnaires will be treated as strictly confidential and in accordance with the terms of the data protection legislation. With your permission, the anonymised data from the questionnaires will be transferred to a secure database to be used for service evaluation and research.

Signing the ‘Client Agreement’ will be taken to confirm that your anonymised data can be used for service evaluation and research. You can withdraw your consent to this at any time by contacting your therapist.

International Clients
The counselling, therapy and coaching supplied by Practically Minded originates in the UK; whichever service(s) you participate in with Practically Minded is therefore subject to UK law and no other jurisdiction.  

Feedback
Please feel free to feedback directly to me on any therapy-related result or change you may notice. You can do this in person in our session together, or afterwards by email - sophie@practically-minded.com. If you feel your comments require a conversation, please email so we can get a feedback meeting scheduled by phone.

I’m open to and grateful for all feedback. It helps me to stay aware of how best to support you as an individual in your unique life situation. It also supports me to continue to improve the therapy I supply going forward.
